I/We hereby undertake to strictly conform to the Bye-Laws of the Association if accepted to Associate Membership; and have pleasure in completing the Questionnaire set out below:
do hereby apply for ASSOCIATED SOCIETY MEMBERSHIP in the Incorporated Masterbuilders Association of Jamaica. I/We hereby agree to pay the fee of $12,000.00 per annum. I/We hereby undertake to strictly conform to the Bye-Laws of the Association if accepted to Associate Membership; and have pleasure in completing the Questionnaire set out below:
